I can't speak to smart carry holsters, but I have used Thunderwear for years and I've never had to adjust the pistol. It stays secure.
I've found Thunderwear to be the most comfortable means of IWB carry for me.
I used to carry a Glock 23 in the front right pocket, two spare full sized magazines in the front left pocket, and in the rear pocket I carried a small keyring sized OC spray, a short ASP baton, and a tactical flashlight. I never had anyone notice that I am aware of.
The holster gives pretty good groin protection as well. It isn't quite as protective as a cup, but it comes close.
